Is that an additional card slot on the exterior of the wallet and, if so, does that mean there are 4 interior slots +1 exterior (5 total)?

No, the Small Executive Wallet only features four card slots and an ID pocket. What you are seeing on the back exterior of the wallet is simply the area of where the sections of leather are stitched together.
